Based on the article appearing above, I can surmise this is not a "freak" accident. From the evidence presented, it is simply a matter of the van driver's desire to get ahead of the vehicle in front. In other words, "Get out of my way, I want to be ahead of you." I think this pretty well describes how too many Sri Lankan drivers negotiate the roads.

Cancel the driver's driving licence for ever, if fount guilty.
Punish that way and cancel 10 licences, remaining drivers could be made to follow the Road Rules.
Enough is enough.

Most school van drivers have no regard for the passengers. Cops are too busy looking in the other direction.

Speed Demons and slow turtles are to be blamed equally.
In the busy Colombo Kandy road every day we see upto 100 vehicles chugging behind a slow moving lorry or a three wheeler.

We should have three lane roads with centre lane alternating every kilometer in between to allow overtaking alternately.
