Girl’s death in Peradeniya

No negligence, rare allergic reaction caused the death

Referring to the tragic and untimely death of a 21-year-old woman who died after being administered a drug via cannula while receiving treatment at the Peradeniya Teaching Hospital on Wednesday (12), Hospital Director Arjuna Thilakarathna denied any medical negligence involving the incident.

Speaking to Daily Mirror, Dr. Thilakarathna said, “She has developed complications after she was administered an antibiotic called Ceftriaxone via a cannula. Ceftriaxone is an antibiotic that is used to treat many kinds of bacterial infections.

“We have used around 2,700 vials containing Ceftriaxone on previous occasions at the hospital. However, no one has been reported with complications until the recent mishap. The death is believed to be due to an allergic reaction. This is a one-in-a- hundred thousand case,” Dr. Thilakarathna.

However, he said that an investigation has been launched into the incident.

Meanwhile, Health Ministry Additional Secretary Dr. Saman Rathnayake told Daily Mirror that the postmortem has determined that the death of the girl had been caused due to anaphylactic shock, which is a rare but severe allergic reaction.

When asked whether the drug has been withdrawn, Dr. Rathnayake stated, “There is no need to withdraw the drug from use as the death is not due to an issue in the medicine but severe allergic reaction developed within her body.”

Meanwhile, the Kandy Magistrate who held the magisterial inquiry into the mysterious death of a 21-year-old girl at the Peradeniya Teaching Hospital issued an open verdict.

In the open verdict, the Magistrate issued an order to refer the specimens of the girl's body to the Government Analyst for a report.

Chamodi Sandeepani Jayaratne (21) who was admitted to the Peradeniya Teaching Hospital on Wednesday (12) to obtain treatment for a stomach ailment reportedly died after being administered a medication via a cannula.

    Usually before receiving an antibiotic for the first time, a TEST DOSE is administered (a negligible amount under skin to check for hypersensitive skin reaction ) to know whether patient is allergic to that antibiotic. It's a standard procedure followed , from the time Penicillin was invented in 1928 and later found to cause allergic reaction in few. Regardless, when the patient is already in hospital, she could have been easily saved, by giving a dose of Epinephrine / anti histamine soon after, realizing, it was anaphylaxis.

    Anaphylaxis is usually an immediate reaction. If not negligence how do you explain not testing for allergy to medicine, giving second vial and not recognizing it's Anaphylaxis. Every hospital / casualty carries Epinephrine / anti histamine. Obviously no one realized (clueless) because cause of death is confirmed by postmortem.

    Even in a small salon, they will do a patch test before using hair color, in case there is an allergic reaction. These hairdressers do not have years of medical education. But Sri Lanka's supposedly qualified doctors and nurses do NOT know to test for allergy before using medication. even after the patient complained previously of feeling burning sensation

    This excuse has become common where all the rarest of medical issues are happening in Sri Lanka. It is common knowledge, specially when giving high dosage of a intravenous medication to check for allergies before administrating. This can be done simply in two ways. Checking directly with patient and family members of historical incidents or medical conditions or administrating a minute dosage and watch for a reaction. It is clear a full blast dosage of a specific reactive medication was directly administered to the veins leading to this death. Obviously, now the whole medical system will manipulate and protect the perpetrators typical Sri Lanka style. That is the culture of this country from the time Tamils were arrogantly killed in the north with complete impunity.

    sad that test for allergy was not adminstered by the nurse.anyway knowing that this patient had a high level infection,where was the ward doctor on duty.He has neglected to pay special attention to a patient who was very ill.If the ward doctor was present he could have administered the antidote for the allergic reaction. Internal admin is rushing to cover up this lapse before the board of inquiry has inquired and issued its decision.

    Before administering drugs, it was the responsibility of medical staffs to ascertain if the patient is allergic to any substances. Patients do not know about such complications as they are usually ignorant about reactions.

    Anaphylaxis is nothing but severe allergic reaction to certain foods, particular insect stings and medications. People in general do not succumb to this common adversity unless and until treatment is delayed. A person dying of this in a hospital is not at all common, because the antidote and other supportive measures are readily available. People can use such terms just to confuse, cover up and make it look serious / terminal.

    Doctors are in most cases thoroughly inefficient and irresponsible in responding to serious cases such as this, the victim's parents are to worry about the girl. Before, administering the drug doctors must check the medical record of the patients to verify the reaction that may respond negatively in some, maybe in rare cases. If the medicine is imported from India, it must be taken extraordinary care to confirm whether it's dangerous-free to administer to a patient. Nowadays, India has been condemned for producing allopathic medicines which are not mostly up to the international standard. Looking forward to taking proper actions by the GOSL to stop importing any medicines from INDIA.

    @ Doctor (fake) 1.Nowhere in the world are all antibiotics tested before administration with test doses. Only one or two with high likelihood of allergic reactions. Even the test dose can cause a fatal allergic reaction. 2.It takes some time for an allergic reaction, not necessarily before the second vial which is given immediately after the first 3.Adrenaline, which they clearly stated was given is the same as epinephrine you mentioned. You seem to think it’s a miracle drug that should cure all allergies. It’s not. If you are so sure that people die of anaphylaxis only due to negligence can you name a single country that doesn’t loose people due to anaphylaxis?

    Same thing happen to my daughter, when she was around 10, she was allergic and Dr. Said she was allergic, still night shift nurse administered the same medicine, the house Dr. Came and treated and she was saved, night Dr. Go and sleep in the house, only for a emergency he is called in, thanks to god my daughter was saved, now she is 30. We never took action against nurse, in Sri Lanka, our system is different, people should be trained to file case against them and those things should be thought in schools and promoted in media.

The learnered doctors may come out with a million excuses for which they are paid for,but within their conscious they all know the truth,and are stubborn to accept it.     Anaphylaxis reaction is possible by injection of an medication. But teaching hospital should be able treat major anaphylaxis reaction within few minutes by supporting airway, breathing and circulation. this death could have been prevented.

    Ceftriaxone was responsible for the highest number of deaths as per a global database (49 cases). Of 20,877 reports, 1205 (5·8%) were related to ceftriaxone; 357 reports (30%) are categorized as serious including cardiac arrest, anaphylactic and anaphylactoid reactions. The high number of serious cases makes it necessary to develop preventive measures for reducing those adverse events. Did the doctors at the hospital have those information? I doubt. What's the justification for such a strong drug. Sri Lankan doctors are very laid-back and slow to react by world standards.

    Doctors are trying to give all kinds of excuses for their own safety. Do we hear similar incidents occurring in other countries as in SL? I live in Dubai and hospitals here, are extremely careful of drugs given to patients. Even if the patient had come to the hospital after taking some home remedy, they note it down for consideration. Doctors ask a number of questions to get details of the medical history of the patient. That's how a doctor should pay attention to patients.
