Send in Engineers to conduct a safety audit. Seems the boiler needs a pressure relief valve.

Examine whether the estate authorities have obtained the boiler licence, maintained the boiler regularly and whether it was operated by a licenced boiler operator. Whatever the actions they take now, its pointless as three valuable lives have been made to sacrifice.

Most of the tea factories have very old boilers under the operation- no safety regulation and no well schedule preventive or predivtive maintenace at all, The administration must be charged for these failure.

This is now over to you Factory Safety Inspectors of the Labour Department to carry out thorough inspection of all factories with boilers to ensure the safety regulations enacted by the factories ordinance are respected by the factory owners and to bring the culprits who turn blind eye to them before the law.
